## 1. Foreign Domestic Helper (Maid / PRA)

### A. Employer Documents
- Cover letter for Visit Pass application
- Authorization letter to representative
- PRA1 application forms (IM.12, IM.38, PRA1, and relevant Appendices)
- Personal Bond with RM10 stamp duty
- Signed employment contract with RM10 stamp duty
- Employer’s and spouse’s:
  - IC copies
  - Marriage certificate
  - Recent salary slips (3 months)
  - Tax documents (2 years)
  - Bank statements (3 months)
  - Employment letters or business registration documents (Form 9, 24, 49)
  - 1 passport-sized photo each
- Documents proving relationship with dependents (birth certs, ICs)
- Medical report for dependent (if relevant)
- Health report from origin country

### B. Foreign Worker (Maid) Documents
- Valid passport (18+ months validity)
- SPIKPA / SKHPPA insurance policy
- Maid protection insurance
- Malaysian-certified health report (blood, urine, X-ray, fit/unfit)
- 2 passport-sized photos

## 2. Foreign Workers in Various Sectors (Construction, Agriculture, Manufacturing)

### A. Employer / Company Documents
- Cover letter using company letterhead
- Power of attorney with company stamp
- Application Approval Review Format (VDR Form Appendix 3)
- Employee list (2 copies) with passport details
- Application Forms: IM.12, Form A1 (Employee Info), Form C (Employer Info)
- Approval in Principle and Employment License under Labour Ordinance (Section 119)
- Company registration documents:
  - *For Sdn Bhd*: Form 9, 24, 44, 49, Certificate of Incorporation
  - *For Sole Proprietor / Enterprise*: Form D, Business name registration cert
- Employer/Manager & Company rep’s IC copies
- Construction contract (if applicable)

### B. Foreign Worker Documents
- Passport copy (valid 18+ months)
- SOCSO registration
- SPIKPA / SKHPPA insurance (not needed for plantation sector)
- Health report from approved Malaysian clinic
- 2 passport-sized photos (6 for Chinese/Vietnamese)

## Common Requirements
- All application forms must be *fully completed and accurately filled*.
- *All documents must be arranged in sequence*.
- Submissions must be made *in person by the employer* at the Immigration Office near the workplace.
